Proposal Information

The following information will be needed to analyze and research the current market for your organization's group health coverage:

  1.   Current census.   Any format may be used as long as it provides, at a minimum, the status of the employee - single, employee and spouse, employee and children, or family and employee's birth date (spouse's if available).  Sometimes the invoice from the current carrier contains enough information.  Additionally, for multiple option plans such as HMO/PPO arrangements, the type of plan each employee has elected should be noted on the census.  If provided, voluntary benefit elections should also be noted.

  2.  Summary of benefits.  This briefly outlines the plan deductibles, copays, covered services, lifetime limits and other major benefits.  Alternatively if you know the plan  number or designation we have plan summaries for the major carriers on file.  Either way, we need to know the level of benefits to be able to match as closely as possible the current level of benefits.  Also if supplementary benefits such as life/AD&D, dental, or disability are provided, the basic outlines or coverage limits are necessary for a complete program review.

  3. Monthly premium.  The current month's payment will suffice.  This avoids meeting to present proposals when the current market is not competitive with the existing health plan's cost.

  4.   Medical conditions.  If you know of any serious, ongoing medical conditions we can avoid initial low cost proposals, with significantly higher rates after underwriting.  This saves a good deal of time.  Cancer not in remission, planned heart surgery, diabetes, etc. would be considered a serious ongoing medical condition.
